I give my son or daughter permission to participate in activities with the St. James Episcopal Church Sunday school and Children’s Chapel program. I agree to grant St. James Episcopal Church permission to record digitally, and on film, videotape, photos, or audiotape, my child’s participation in St. James Sunday school and Children’s Chapel Events. I further agree that any or all of the material recorded may be used, in any form, as part of any future production(s) made by St. James Episcopal Church-Wichita. I allow my son or daughter’s photos to be used on St. James Episcopal Church website, Facebook, Twitter and other social media accounts. This permission form is good for the year of August 31, 2024 - August 30, 2025

Note: St. James will not publicize names of students on any social media accounts including Facebook. Instagram and Twitter.

*Note: There is no fee for your child associated with Sunday school. If you would like to make a donation towards Sunday school there is a donation box located outside the classrooms. Thank you!
